Whether your child needs nursing or therapy services, we are here to provide the best possible care. Our dedicated staff will determine the right course of action based on your needs.
Nursing Referrals:
For patients transitioning from hospital to home, a liaison from our agency will visit you in the hospital. At this visit, our team will meet with you to understand your family’s unique needs. We will introduce you to our services and discuss ways we can assist you in the home. We will evaluate your child and obtain clinical information to begin the insurance authorization process.
Once insurance authorization has been received, an initial home visit will be scheduled.
Therapy Referrals:
Across Health will obtain information from you and begin the authorization process. Once authorization is obtained an evaluation visit will be scheduled. At this visit, admission paperwork will be completed if your child does not already receive services from us. Once the evaluation is complete, authorization will be obtained for ongoing visits.
